UNION COUNTY, Ohio — A tragic two-vehicle crash on Sunday evening has left a 17-year-old girl dead and four other individuals injured. The incident occurred around 7:24 p.m. on Hyland Croy Road near Johnston Drive, drawing a swift response from the Ohio State Highway Patrol (OSHP).

Teen Driver Identified as Claudia Phan

According to investigators, the crash involved a Kia Forte operated by 17-year-old Claudia Phan of Dublin, who was traveling northbound on Hyland Croy Road. Preliminary findings from OSHP indicate that the Kia traveled left of center, colliding with a southbound Ford Explorer driven by 37-year-old Jason Lyle of Lewis Center.

After the initial impact, both vehicles veered off the right side of the roadway.

Tragically, Claudia Phan suffered fatal injuries and was pronounced dead at the scene. Her passing has left her family, friends, and community in profound grief.

Four Others Injured in the Crash

The collision also resulted in injuries to four occupants of the Ford Explorer:

• Jason Lyle, 37, the driver, was transported to Riverside Methodist Hospital with serious injuries.

• Ashley Lyle, 34, the front-seat passenger, was taken to the same hospital with minor injuries.

• Two juvenile passengers in the Explorer were transported to Nationwide Children’s Hospital, also with minor injuries.
